Helping people with serious mental illnesses to live longer

Our Mental Health Physical Health programme has now been adopted in 164 Clinical Commissioning Groups (CCGs) across England, covering more than 600 organisations including primary care, mental health trusts, community health providers and prisons.

People with a serious mental illness (SMI) are at risk of dying prematurely, in some cases 20 years earlier than the general population. Many of these deaths are preventable if conditions like high blood pressure, diabetes and cardiovascular problems are identified and treated early.

Our Mental Health Physical Health programme supports clinicians in conducting physical health checks on patients with SMI. It is continuing to spread across the country and is helping to tackle the issue of people with an SMI dying prematurely from identifiable and treatable physical health issues.

An updated Mental Health Physical Health Review Template has been created to align with NHS England guidance. This includes:

  • updating to SNOMED coding
  • improved capture of QOF indicators.

The template should be used by any GP practice who has SystmOne as it will help achieve QOF metrics as well as improving the physical health of SMI patients.

The template can be found in the .zip folder here  along with import instructions.
