Remote Monitoring and Virtual Wards Resource Series: Stage 4 – Embed

Written by: Nicola Chicken - 2nd February 2023

We are nearing the end of our series of blogs including some useful resources to support our NHS colleagues interested in implementing remote monitoring solutions and virtual wards. Over the past two years, we have been working with health and care providers across our region to support their ambition to roll out digital remote solutions that will transform the way we care for people.

Our blogs have explored the key stages in the journey to deploy these solutions, the challenges and the benefits they can bring to patients and the NHS. The fourth in our series of resources  brings us to the implementation process stage of ‘Embed’. This is the point in the journey where your service has gone live and patients, families, carers, health and care professionals and the system begin to benefit from the redesigned pathway.

To date we have published our resources covering the Start stage, Engage stage and Deliver stage that include guidance on topics such as the end-to-end implementation process, writing a business case, digital solution selection, setting up an evaluation, benefits measurement, training and IT set up.

Once your service has gone live, there are a range of activities to consider ensuring that evaluation of the service can be undertaken, progress such as activity levels are tracked and reported, the service is continually reviewed and improved based on stakeholder feedback and that all people involved in the service are supported with the new processes, systems and technology. The overarching aim of these activities should be to make the redesigned service and pathways as effective, efficient and safe as possible for all involved, as well as capturing lessons for improvements and future implementations.

To support you at this stage we have created the infographic below covering a range of activities to complete at this stage of the implementation process.

We hope that you find these resources useful, but we are also keen to hear any feedback or queries you may have. Please feel free to contact Nicola Chicken at nicola.chicken@yhahsn.com
