Helping patients receive medicines support on discharge from hospital

The Yorkshire & Humber AHSN has been working closely with Hull University Teaching Hospitals NHS Trust to implement the Refer to Pharmacy programme across the city to enable cardiology patients to receive medicines support following discharge from hospital.
Hull University Teaching Hospitals NHS Trust has now successfully referred 132 cardiology patients through this service. Due to its success Hull University Teaching Hospitals NHS Trust are now looking to expand the service to other departments across the hospital.
